Authorize a Representative – signature page - Suggestion

I just had a client balk at signing the Aurhorize a Representative page because my name is nowhere on that page.

 

Yes, my RepID is there, but how does my client know that that RepID is MY identification number?

 

Is it too much to ask that, when a client is being asked to sign a form, that the form has enough information for the client to have confidence in signing the form?

 

I suggest that it would be a good idea to have the representative name (whether individual, business, or Group) on the form so that the client knows who they are authorizing

I think that this is CRA's form, not Profile's. But I've been surprised, too, that neither my name nor my firm's name appears on the form. I'm glad most clients do not read this form before signing; if they were, I'm sure some would have seconds thoughts before signing it.
