Where to find the Ohio IT 1140 form for an Ohio Trust?

Where can I find the OH Form IT-1140 for an Ohio Trust in Proseries Professional?  When I called Proseries support, they said it is only available through the Federal 1065 or Federal 1120S, not the 1041.

    Yes but I was wondering if there was an electronic workaround within Proseries.  I couldn't find one for a trust.  Old fashioned way it will be.  Thanks.
