Where can I find the KY ammended tax form

I am ammending a tax and I cant find the state form to ammend that too .  My state is Kentucky.

does anyone know where i can find it?

To note an amended return you use the same Form 740 and just a bit below the top of the Form 704 is a box to mark the return as amended.
