Overtime not reported on W2

The overtime amount is not reported on box 14.  When completing Part VII, Deductible Overtime, I get a calculated deductible overtime amount, but the qualified overtime 14e for  Sch 1-A is $0.  MAGI is well below the $300,000.  Why?
