Input Form 1099-R Error

Trying to input Payer's zip code from 1099-R, but no luck.  Looked up address for Matrix Trust Company (Pheonix, AZ) online, and it is the exact same as shown on 1099-R.  Double checked and triple checked zip code input, but still unsuccessful/-.  Please HELP!

It's spelled Phoenix, but you shouldn't have to enter the city before entering the ZIP Code -- last time I checked it's 85021, with a zero not a capital O, a couple miles south of my office.
