How can I Efile a 1095-A with no amounts in columns B & C?

I have a client that received a 1095-A stating what she had paid for 5 months and the problem is that the HealthMarket did not pay anything towards her portion of the insurance, she paid the full amount each month for 5 months, so therefore columns B & C both have zeroes which means they did not qualify for any payment help and I'm getting errors?  Any solutions to this where I can e-file their return?

Like Lisa said you have to find the values to put in column B, and column C would have all zeros
