Gift Tax Notice of Spousal Consent Signature Page

Does anyone know where the spouse signs the gift tax return giving the spouse consent to split the gifts. The letter says line 18 of page 1 but that is not correct.

The form changed this year.

You have to have a statement for the election and spouse signs that.

The more I know the more I don’t know.
