Amortization of Domestic Research and Experimental costs

I am getting an explanation statement titled "Amortization of Domestic Research and Experimental costs" and I have no idea what triggered this form. It only appears when the tax return is printed. It does not appear in the "Forms In Use" list. Anybody encountered this issue.
