8915 form Lahaina Wild Fire Disaster

Client wants to use the 8915 form to allocate his 1099R distribution over three years for

rebuilding his house from the Lahaina Wild disaster fire - having problems getting it to allocate

the distribution over 3 years after inputting the data on 8915-F form.  Any advice?

Did you fill out the 1099R worksheet ? don't forget to fill out the Qualified Disaster Distributions Worksheet, about 1/3 way down on 1099R worksheet
