Why is my preparer information showing on the 4868 instead of the clients address

My name and address is appearing after the clients name instead of the clients address after I generated the 4868 extension,  the NY extension appears to be fine.

Preparer address is used by default.  To show your client's address, you need to enter 2 for the following line(s): Reply: 1=firm address, 2=client address [Override]
