Why is form 1040x blank and 1040 complete?

We are required to e-file an amended return to add form 8962, but the 1040X is blank.  How do we fix this?

I use lacerte so I may be barking up the wrong tree, but in the interest of helping (and in the interest of barking up the wrong tree) give this a try:

In Lacerte, in the 1040X screen, there is a box where you have to put a 1 to indicate 1040X and another box to indicate 540X (California, my home state).  Is there such a box for your software?
