Why is FIRM address being printed on 4868 Application for Extension

I'm previewing the 4868 application for extension which correctly shows the client name and social, but the ADDRESS is printing as the FIRM / Preparer address.  When I click it, it takes me to the client profile address which of course is entered properly and not the same as the preparer address. 

How to fix?
