What is checked in Box 3?  You might have to file Form 8853.

    Deductions > HSA/MSA/LTC Contracts.... > L/T Care Insurance Contracts (8853)

    Do some research to see if this is even necessary. Start with the instructions that actually are on the form itself, then look at the instructions for producing the form https://www.irs.gov/pub/irs-pdf/i1099ltc.pdf
