Where do I report advance premium tax credit received for health insurance received through State of NJ marketplace? Should I have received a form?

If you mean insurance thru the Marketplace or Health Care .gov, the taxpayer should have received a 1095-A, which goes into the tax return and calculates if there is a credit or repayment amount on form 8962.
