Is ID Form 402 available?

I'm preparing a multi-state return and see this message regarding the Idaho return: "The State of Idaho requires a PDF copy of Form 402 Idaho Individual Apportionment for Multistate Businesses to be attached to the return when there is business income on Form 43." I don't see the Form 402 listed anywhere. Is there a way I can get it in ProConnect, or do I have to pull it from Idaho's tax commission and manually create it?

ID MeF does not support e-filing of Form 402 but allows the form to be included as an attachment on an e-filed return.
