How do I eliminate critical diagnostic for form 5329 Waiver of Tax on Birth or Adoption Statement?

I posted this several days ago and did not get an answer. I input the child's Name, Age and SSN but PTO is still returning a critical diagnostic indicating that I need to input this information. What am I missing?
