Reporting Dividends from a T5/NR4 Summary

My Client has a T5/NR4 Summary of Investment Income and Expense that is showing an amount for Total Eligible Dividends from Canadian Corps which are listed under "PAID TO YOU", but does not have any other slips. How do I report this? Any advise would be appreciated

May be on S125 under GIFI 8089 section, like 8096. There is also schedule  S3  for dividends received, you can verify if it applies to you.  In case of doubt, you should ask CRA.
