Profile T1: Custom Document

Hello,

Using Profile-T1 (2020.3.0).

 

I like to create a custom document having text and fields (client name, address, etc.), please see attached sample.

 

1. How to create a custom document in Profile?

2. How to associate this document with form-RC71 (i.e. every time we print form-RC71 this custom document is also printed automatically)?

Need help with the template-editor:
 
1. How to get bullets (numbers, letters, bullets) in the template-editor?
2. How to get the field to sit on top of a line in the template-editor?
3. Depending on the filer's gender, how to get "his" or "her" to appear in the text in the template-editor?
