Beneficiary Details: Removing Unnecessary lines from the name column

The name column of the T3 beneficiary details contain a number of "Unknown" lines that I need to either hide or remove. I have tried, but is unable to edit for removing them. These additional line are causing a failure in the e-filing process.

Has anyone encountered this issue?
