Where is the Lacerte Input for NYS Form IT-636?

Form IT-636 shows as a Form on NY but when you right click to jump to input it only says Zoom In Zoom out - I can't find the input fields for the form

I found this as an override in the individual module

I went to Manual Entry Forms and found it is not yet available.
