What form do I attach Sch D transactions statement that I had input just summary on Sch D- 1041, to mail to the IRS?

how many transactions are not covered? Do you really need to attach anything?

I just listed the summary of the short and then long term sales on Sch D & checked the box "Summary from attached statement", which would generate form 8453 to mail in the 1099B transactions to the IRS when preparing a 1040.  This form is not available on a 1041 - so what form is used to mail in the 1099B transactions to the IRS?
