I just need to know if a QCD is taxable to the exempt church who files 990-T only.

No. Churches (and all tax exempt organizations) do not pay tax on donations received.

If they got a 1099-R it wasn't done as a QCD, but it is still not taxable.

The more I know the more I don’t know.
