Oregon MC & MET 40

I have attached the OR-40 PDF for MC-40 & MET-40 in Lacerte and checked the boxes on screen 4.1, but E-file is still not adding it and gives error 57483. Any suggestions to get the e-file wizard to see it or stop the error code?
