Question

I believe that the information from a form 5498 should be reported on the HSA/MSA screen but that screen has no place for box 2,3,4 and 5 from the 5498 to be entered?

    Type Form 5498 into the 'search' box (upper right corner) while in the program.  There's a help article that appears.
