How can a Federal Supplemental Information form be eliminated from subsequent 1040 filings, seems to want to continue to carry forward when it is no longer applicable.

I think you might be seeing a NOTE, Screen 47 of the Individual Return. If that is the case, just look at Worksheet or Statement and select Delete from the bottom left side.
