Can anyone let me know how to file Oregon's Metro Supportive Housing Services Personal Income Tax?

The inputs for the OR-VSI form are on screen 3 > Oregon Miscellaneous section.
To file the Metro SHS income tax, you look at the various local tax forms under screen 54 Taxes, then choose the last item in the list on the left Oregon Local Forms MC-40/MET-40. MC-40 is the Multnomah County PFA tax (preschool for all) and MET-40 is the Metro SHS tax.
